Are ALL disciples of Christ believers and thus saved?

So, what's the difference between a believer in Christ and a disciple of Him? Are they BOTH necessarily saved? What is a saved believer in Christ according to the Bible?

Many churches are filled with people who self-identify as Christians that claim belief in Jesus, and yet they are not saved? What, you say - how is THAT possible?
